I understand that the information provided on the inVolve platform should not replace medical advice and I should direct any questions to my healthcare team.
I understand that vorasidenib has not been approved by the The Therapeutic Goods Administration (TGA) in Australia or the Medicines and Medical Devices Safety Authority (MEDSAFE) in New Zealand.
I understand that any side effects experienced while taking vorasidenib should be reported directly to Servier via the REPORT A SIDE EFFECT link at the bottom of the page, or directly to medinfo.au@servier.com
I understand that any possible side effects or other incidents related to the use of vorasidenib that I communicate to Servier via the inVolve platform or in other correspondence, irrespective of causal association, including the situations below will be followed up by Servier and reported to the TGA, I agree to Servier medical information contacting me to collect additional details related to these event.
- Exposure during pregnancy (maternal and/or possible transmission via semen) or breastfeeding
- Overdose
- Intentional abuse or misuse
- Medication error
- Lack of therapeutic efficacy
- Occupational exposure
Use of personal information
I understand and agree:
- That Les Laboratoires Servier (Data Controller), Servier Laboratories (Aust.) Pty. Ltd. (Servier Australia) and their subcontractor, Imagine if Health, are collecting and using my personal information as necessary to provide access to the inVolve platform.
- My personal information (name and email address) will be shared with relevant Servier Australia staff only if I choose to engage with them via the platform and I will be prompted to approve this.
- That other than as set out above, my personal information will otherwise be collected, disclosed, processed and stored in accordance with Servier Australia’s Privacy Policy that can be found at www.servier.com.au/privacy-policy
- To my personal information being kept for 2 years after I delete my login to the inVolve platform.
- That I have the right to see, change, delete, object to, restrict the use of, and receive (get my personal information in a readable format) my personal information.
- That I may lodge a complaint regarding the protection of my personal information with the Office of the Australian Information Commissioner: GPO Box 5288, Sydney, NSW 2001. Tel: 1300 363 992. (add nz details)
- Data will be collected and stored by inVolve platform provider Imagine If Health on behalf of Servier Australia.
